ENDOVASCULAR REPAIR OF TRANSPLANT RENAL ARTERY ANASTOMOTIC PSEUDOANEURYSM USING FLOW DIVERTER

Purpose :
Transplant renal artery (TRA) anastomotic site pseudoaneurysm (PSA) are uncommon and are associated with high morbidity and mortality in the event of rupture, infection, graft dysfunction and graft loss. Recently the endovascular treatment is the preferred option in view of its minimal invasiveness. PSA are commonly managed using coils, stents, and liquid embolic agents. Flow-diverter (FD) are specially designed stent to maintain laminar flow in the parent artery and side branches thereby promoting the thrombosis of the aneurysm.
Materials & Methods :
We report the management of a renal transplant recipient who presented later after ten months with fever, dysuria and deterioration of renal function. Imaging revealed TRA pseudoaneurysm at the anastomotic site. After the infection was controlled with appropriate antibiotics. We successfully treated the PSA using an intracranial FD Surpass Evolve (SE) by deploying the device across the neck of PSA with preserved distal flow in the parent arteries.
Results :
Follow-up CT angiography at one month revealed complete thrombosis of the PSA, equivalent to O’Kelly-Marotta [OKM] Grade D.
Conclusions :
Surpass evolve FD can be used safely to treat transplant renal artery PSA regardless of their location, when anatomic factors may limit the efficacy of classic endovascular techniques. However for further validation, large prospective studies are required.
